Please fill out the form with your child's name not yourself.

Parent Forms & Waivers for your child to attend the Strawberry Point 5th Grade Graduation Pool Party. Your child may not swim unless these forms are signed and completed.

No Pool Floaties or water guns allowed. This party is for 5TH GRADERS ONLY, no younger/older siblings.

All children will have to take a swim test. The swim test is 1 lap swimming, and 1 minute of treading water. If your child does not want to take the swim test, that is ok! They will have to stay in the shallow end of the pool or in the teaching pool. If a child does not pass the swim test, they will have to stay in the shallow end or the teaching pool, a parent may have to get in the water with them.
